Finegreen are currently supporting a local based hospice that provides free, compassionate palliative care and support to patients and their families. In recent years, due to demand within the local area they have expanded their service provision and are now working more so in the community aswell as providing hospice based care. The client is looking to complement the existing team with a senior level qualified social worker, a candidate that has worked within similar settings within the community as will be in the majority but will be dealing with families and loved ones of patients and looking after the best needs of those involved in the patients journey. This person will be joining a well-established, embarking on expansion this is an opportunity to help the senior team carve out what this role will look like on a permanent basis for the hospice. General Duties: Looking at the current service provision, understanding how the social worker role will form part and add value to the team and evolve the current service offering to ensure they really do have a fit for purpose, MDT moving into the future; Develop strategies for identifying patients and family members who may benefit from support, signposting, information or advice of a Social Worker; Triage referrals into the service, undertake assessments of needs and allocate to appropriate support including referral to specialist services where additional mental health or other needs are identified; Work collaboratively with other agencies and services to ensure a clear pathway for referral and signposting is in place to ensure patients and their families access the most appropriate service for their level of need; Attend in-house weekly IPU and FSS MDTs, and other in-house and community based specialist palliative care multi-disciplinary team meetings when required; Support the Family Support Service Manager in the update and debrief of FSS Spiritual Care Volunteers visiting weekly on IPU, to maintain and support a structure and process for the supervision of volunteers according to their level of skill and experience; Assist in planning and implementing systems for evaluation, monitoring and development of the role of the social worker in supporting patients with life limiting illnesses and their families/carers; Ensure effective and compassionate client safety, underpinned by a culture of openness and transparency and continuous learning and improvement; Maintain a defined caseload including triage of referrals in to FSS, assessments, ongoing client work, and Supportive Calls work, as agreed with the Family Support Services Manager. The ideal candidate: Current registration as a Qualified Social Worker in the UK; Professional experience of external Social Care systems and pathways, and how and where to signpost to external services; Knowledge of legislation & national guidance in relation to social work and how this is applied to best support issues faced by patients and families in end of life care and bereavement care; Knowledge of relevant legislation and best practice relating to safeguarding patient/client safety, safeguarding adults at risk, mental capacity and deprivation of liberty safeguards, human rights, mental capacity and the Children’s Act.
