Job Overview: We are looking for an experienced and dedicated CHC Senior Nurse Assessor to join our Adult Locality Team. As a Senior Nurse Assessor, you will be responsible for conducting Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. You will act as a case manager, ensuring that clients receive high-quality care in a suitable environment while working collaboratively with multidisciplinary teams. Key Responsibilities: * Undertake comprehensive CHC assessments and reviews in line with the National Framework for NHS Continuing Healthcare. * Manage a complex clinical caseload, including new applications, unscheduled reviews, and fast-track cases. * Work closely with healthcare providers, social care teams, and independent sector colleagues to ensure appropriate care. * Present assessments to the CHC panel and provide professional advice on healthcare funding. * Communicate effectively with patients, families, and other healthcare professionals. * Ensure all care packages provide value for money and meet clinical needs. Essential Requirements: * Registered Nurse with a current NMC registration. * Proven experience in conducting healthcare assessments, particularly within CHC. * Strong communication, negotiation, and conflict management skills. * Ability to work autonomously, managing your caseload and prioritising tasks. * Understanding of the National Framework for Continuing Healthcare and NHS Funded Nursing Care. Desirable: * Experience in managing budgets and understanding financial processes. * Knowledge of safeguarding policies and experience in handling vulnerable adults. * Experience working within a multidisciplinary team and leading on case management.
