Responsibilities: * Deliver personalized and compassionate care to individuals in their own homes. * Assist with daily living activities, including personal hygiene, dressing, mobility support and administering medication. * Monitor and maintain the well-being of clients, reporting any changes to the healthcare team. * Accurately document observations and activities. * Engage in meaningful interactions to support clients’ social and emotional well-being.
