Job Title: Senior Community Mental Health Nurse – Dementia Service Tripod Partners are currently recruiting for a Senior Community Mental Health Nurse to join the Dementia Service in Shropshire. This role focuses on the assessment and diagnosis pathway within the service. Key Responsibilities * You will support the management of referrals into the Dementia Service, working closely with the operational lead and Nurse Practitioners to ensure efficient processing. * Your role will involve reviewing referral information, gathering additional details if necessary, and coordinating with primary care, other services, patients, and their families to ensure smooth progression to assessment. * This position requires experience in dementia care, particularly in assessment and diagnosis, with a preference for experience in dementia or older adult services. * The role allows for a blended working approach, with a mix of home-based work and some office presence, particularly during the initial training and induction phase.
