Job summaryWe have an exciting role in the Geriatric Division for a Discharge Pathway Coordinator on Queens Frailty Unit. Care of the Elderly is currently going through a process of change to ensure we meet the on-going needs of our elderly patients and to provide an excellent standard of care every time and every day.We are looking for an enthusiastic care pathway facilitator with post experience working in elderly care and has a passion for delivering safe discharge and reduce the length of stay of our elderly patient, putting the patients at the centre of our PRIDE values and promoting good patient experience.The post holder will work in a busy acute ED setting for frail and elderly patients with frailty score of 6 and above and will ensure that all complex discharges are dealt with in a timely manner within the agreed timescales and patients are discharged safely.The postholder will be required to establish and maintain effective working relationships with both Trust staff and relevant external agencies and advice the senior sisters and charge nurses /matrons/Lead Nurse, PALS and Family Liaison on areas of risk to the organisation and take necessary actions as appropriate to ensure that patients, relatives and carers who raise complaints regarding poor discharge/discharge alert are dealt within a sensitive, effective and confident manner in line with Trust discharge Policy. Main duties of the jobThis post is part of the team of patient pathway coordinators within Care of the Elderly Speciality in Queens to facilitate discharges. The post holder will work collaboratively with the team on the Queens Frailty unit nursing staff in all clinical areas where there are patients to coordinate discharge plans.You will assist with all patients in their care and work under the direction of the senior sister and medical staff to ensure that patients receive optimal discharge plans within agreed time framework. You will work as part of the team across all Care of the Elderly Wards. The post holder will attend bed meeting and work across trust to provide support for acute medicine and coordinate discharge arrangements Accurate record keeping and documentation is an essential component of this role as effective communication with a wide range of healthcare professionals and patients. This should be countersigned by trained nursing staff. The postholder should be supervised by the senior nursing team.
