Community Care The Community Care department focuses on assisting individuals in accessing care services at home, in institutions, or through health and housing services. Collaborate with the Board to achieve performance targets. Manage all aspects of Community Care work and provide guidance to team members as needed. Implement recommendations from file reviews and ensure compliance with file management standards. Proven supervisory experience and ability to work to performance indicators. Excellent client care, communication, and networking skills. Experience in both publicly and privately funded Community Care work, including Wills & Probate. Advocacy experience is advantageous.
