Leaving Care/Personal Advisor

BBS Recruitment is an independent recruitment agency for the transport and logistics sector, supplying to a variety of clients across London, Hertfordshire and surrounding areas. We also recruit for Social Care and Public Sector roles. We are currently recruiting for an experienced Leaving Care/Personal Advisor to work for our client based in Tower Hamlets and surrounding areas. Duties & Responsibilities: * Visiting service users * Meet with foster carers / keyworkers and significant others to ensure the service users are happy, settled and able to focus on life goals * Arrange review meetings and support the service user to express their views and wishes on plans for their future * Support the client in all aspects of their health, including arranging health and dental checks, and make sure they have any immunisations need, access to fitness, and access to emotional and psychological support * Support the service user's education, * Work with the service users to develop life skills required to manage adult life * Ensure the service users receive advice and guidance with jobs, careers and training * Support you the service users in all aspects of their health * Organise a Pathway Plan review meeting every six months (minimum) after the service users have left care to update their pathway plan * Support the service users to access appropriate independent housing when the time is right and ensure they know how to manage a tenancy Working time: 07.15am - 17.00pm, Monday to Friday Requirements: * Previous experience in leaving care service for local authorities * Enhanced DBS on the update service * 5 Years Work Reference If you have the relevant experience, please apply with your CV
