An exciting role has recently become available for a Qualified Social Worker at a local autority based in the Nottinghamshire area to support their adults team.

Manage a complex caseload, assessing, planning, and reviewing individual needs to promote independence. Conduct assessments using various health and social care tools, including continuing healthcare evaluations. Identify and facilitate access to Reablement services, promoting a Strength-Based Approach to enhance independence and well-being. Utilise community resources and individual strengths to maximise preventative support and delay long-term care needs. Coordinate ongoing service provision, ensuring effective collaboration, monitoring outcomes, and undertaking safeguarding responsibilities.What we are looking for in a Social Worker:
Social Work England registration Eligible to work in the UK Hold a full UK licence Post-qualified experience Degree level or equivalent in Social WorkWhy Liquid Personnel?
