We are seeking a dedicated and experienced Care Plan Manager to join our team in South England. The successful candidate will be responsible for developing, implementing, and managing personalized care plans for our clients to ensure they receive the highest standard of care. The role requires strong organizational skills, excellent communication abilities, and a compassionate approach to care.
Key Responsibilities:
- Assessing clients' needs and developing tailored care plans.
- Coordinating with healthcare professionals, clients, and their families.
- Monitoring and evaluating the effectiveness of care plans.
- Making necessary adjustments to care plans as clients' needs change.
- Ensuring compliance with health and safety regulations and standards.
- Documenting and maintaining accurate client records.
- Providing support and guidance to care staff.
Skills and Qualifications:
- Proven experience in care management or a related field.
- Strong understanding of healthcare regulations and standards.
- Excellent organizational and time-management skills.
- Exceptional communication and interpersonal abilities.
- Compassionate and patient-centred approach to care.
