District Nurse

We are actively seeking skilled District Nurses to join our team supporting NHS patients withing their own homes and the community. Duties: Community Nursing Care: Providing essential nursing care to patients in their own homes, including wound care, administering medications and managing chronic conditions. Patient Assessments: Conduct assessments of patients' health needs, develop care plans and monitor their progress. Health Education: Educate patients and their families on managing health conditions, medication, and lifestyle changes to improve health outcomes. Collaborative Care: Work closely with GPs, other healthcare professionals, and social care teams to deliver integrated care.
