Job summaryCome and join Leicestershire Partnership NHS Trust, where we offer a wide variety of challenging and rewarding roles within a supportive, innovative and dynamic work environment!An exciting opportunity has arisen for enthusiastic individuals to join our friendly primary care Mental Health Facilitator team as health care support worker.Mental Health Facilitator service provides Annual Mental Health and Physical Health review to individuals with Severe Mental Illnesses across Leicester, Leicestershire and Rutland.We have 2 permanent positions available within Leicestershire (county area) , working 37.5hours per week, Monday to Friday.The principle role of the Senior Healthcare Assistant is to assist the healthcare team in the provision and delivery of programmes of patient care under the indirect supervision of registered practitioners.The health and wellbeing of our team members is very important to us and we offer a robust induction period, as well as regular supervision and support. Main duties of the jobThe successful candidate will bewill need to be a good team player, with excellent organisational and communication skills and be able to work on your own initiative and prioritise your work effectively. Must have phlebotomy training or willing to complete the training upon appointment.Primary responsibilitiesSupporting clients/service users in maintaining their health and wellbeing,Taking physical (including blood tests)or psychological measurements from named clients/service users as delegated by the team;Supporting clients/service users in maintaining their health and wellbeing;Undertaking specified and delegated clinical and therapeutic activities;Under the supervision of healthcare professionals assist the healthcare team with the planning, organising and provision of clinical care;Ensuring high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals;Assisting Registered Professionals in the implementation and evaluation of individualised care plans;Accurately recording patient information; andProvide support and supervision to other colleagues and learners as appropriateTo demonstrate the Trusts values in everything you do in the work environmentWork autonomously and be a car driver and owner.
