As a Dementia Care Manager, you will:
Lead and manage the dementia community in the home. Deliver dementia-related learning and support to team members and relatives. Develop and implement Individual Care Plans, Risk Assessments, and Daily Assignment Sheets. Ensure compliance with CQC regulations and Hallmark standards. Act as a coach to team members, ensuring resident needs and preferences are met. Communicate regularly with families, GPs, and other professionals. Lead the delivery of the Dementia strategy at home level. Work with the lifestyles lead to ensure a suitable programme of activities.Package and Benefits:

NVQ 4 or 5 / QCF 5 in Health and Social Care or willingness to work towards it. At least 4 years’ experience in long-term care of the older person, including dementia care. Detailed knowledge of care plans and end-of-life care. Supervisory and management experience. Academic qualification in Management and Leadership or equivalent experience. Knowledge of adult abuse, safeguarding, and CQC regulations. Strong IT, communication, and organisational skills.If you have experience as a Dementia Care Manager, Dementia Care Coordinator, Dementia Specialist, Unit Manager, or Senior Care Manager, this role could be perfect for you. Your expertise in dementia care will be highly valued in this position.
