Diabetes Nurse Prescriber – Community-Based

Tripod Partners are currently recruiting for a Diabetes Nurse Prescriber to work with the South Sefton Diabetes Team based. Key Responsibilities * You will provide expert diabetes management, including prescribing medications, and offering specialist care to patients in both clinic settings and during home visits. * Collaborate with the multi-disciplinary team to ensure comprehensive diabetes care and support for patients in the community. * Initially, support and guidance will be provided via shadow shifts to ensure a smooth transition into the role.
