Qualified Social Worker | Adults Hospital Team |London Borough of Camden Qualified Social Worker required to join a statutory Hospital Discharge in line with Discharge to Assess Team Office base: University College Hospital London To work Friday, Saturday and Sunday and Bank Holidays. Friday - based in the hospital. Sat and Sun - mainly remote working. Candidate should have experience of Hospital Discharge in line with Discharge to Assess. Knowledge and skills in line with Care Act responsibilities.
