Ty Anwen is a residential mental health service in Caerphilly. We provide person centred support for up to 16 individuals with serious and enduring mental illness and complex needs which may include support with self-harm.The team at Ty Anwen work to a recovery model of care and offer support that provides effective therapeutic boundaries. They provide clearly identified outcomes, thereby working towards recovery and facilitating independence.The role of the Complex Care Project Worker is to empower and support individuals we support, working flexibly to improve performance and outcomes for people who use our service. You will work as part of a team to develop, implement and review high-quality person-centred care and support plans based on individual needs, preferences and aspirations when required.As a complex care project worker you will be involved in day-to-day support including;Supporting individuals to live independently within a care setting.Support, prompting and administering medication.Engagement in social activitiesCompleting and updating support plans, risk assessments and risk management plans.Carrying out supervisionsCompleting assessments from referralsBe responsible for the service in the absence of the manager.Undertake relevant training for the role.Liaise with Social Services, Benefits Agencies, Housing department and other relevant agenciesOrganising group activities with the individuals at Ty Anwen or in the community Providing emotional support to the individuals What skills and experience do you need? A genuine desire to help people and make a difference.Have or be willing towards completing a QCF in Health and Social Care level 4 or above.An understanding of the challenges that many of our service users faceKeen listening and communication skillsEmpathy, compassion and a caring natureAbility to complete and regularly review relevant paperwork such as support plans and risk assessments.
