An exciting job role has recently become available at Hammersmith and Fulham. They are looking for a new member for their Hospital Discharge Team. You will manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, and being part of a duty Rota. You will deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible. You will engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks.What we are looking for in a Social Worker: Social Work England registration Eligible to work in the UK Hold a full UK licence Post-qualified experience Degree level or equivalent in Social WorkWhy Liquid Personnel?
