Locum Social Worker - Hospital Team

Social Worker - Hospital Team South Wales based local authority are currently seeking a Locum Qualified Social Worker to join their team within the Initial Support Service. This role focuses on supporting both hospital discharges and community referrals. Key responsibilities include: Conducting initial assessments, care plans, and reviews. Participating in MCA (Mental Capacity Act), Best Interests (BI), and DST (Decision Support Tool) meetings. Planning support for individuals in their own homes. Assessing eligibility for care placements.The team holds cases for a maximum of 12 weeks, with a fast turnover, allowing you to complete tasks efficiently and move cases forward without long-term involvement.
