We are seeking a dedicated and experienced Care & Support Coordinator to join our team. In this role, you will be responsible for ensuring that high-quality care and support is provided to our service users by leading, supporting, and coordinating a team of Care and Support Workers. You will work closely with service users and their families to develop personalised care plans, ensuring all risk assessments are completed and up-to-date. Key Responsibilities: Complete care and support plans and risk assessments with service users, recognising individual needs and varying abilities. Supervise and support Care and Support Workers to deliver high standards of care in accordance with agreed plans. Allocate work to Care and Support Workers and monitor performance. Ensure Care and Support Workers have the necessary knowledge, skills, and equipment to perform their roles safely and effectively. Conduct Moving and Handling Risk Assessments and environmental/lone worker risk assessments to ensure safety. Identify and address the training needs of Care and Support Workers. Ensure regular 1-to-1 and group supervision meetings, in line with organisational policy. Oversee the implementation of care plans to promote and maximise independence for service users. Liaise with health and social care professionals to ensure effective delivery of care and support plans. Monitor and audit service user records, ensuring accuracy and appropriateness. Address any unsatisfactory work activity and respond to emergency situations appropriately. Assist with personal care needs and medication administration in line with care plans when necessary. Ensure adherence to standards of dress and conduct for Care and Support Workers.Essential Skills & Experience: Proven experience in a care supervisory or coordination role. Knowledge of care planning and risk assessment processes. Strong leadership and communication skills. Ability to manage a team and ensure high standards of care delivery.
