Clinical Lead - Transfer of Care Hub

Detailed job description and main responsibilities Develop detailed and complex technical understanding of discharge to assess pathways and hub processes to lead service delivery from a point of professional expertise and competence. Work with the Integrated Intermediate Care Coordination Service (ICCS) Manager in assuring safe and effective service delivery including responsibility for data reporting. To deputise for the Integrated Intermediate Care Coordination Service (ICCS) Manager where necessary including attendance at required system or NWL forums. Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity. Person specificationQualifications Essential criteria First level degree or equivalentEvidence of further relevant coursesEquivalent significant experience of managing discharge and patient flowHCPC Registered Therapist / First Level Registered Nurse or Therapist currently registered with relevant professional body Desirable criteria Management training and qualificationMentorship courseLeadership courses/qualificationsExper Essential criteria Has the required level of knowledge of area of specialty and be able to apply that knowledge in order to promote the health of the patients.Extensive experience at minimum of Band 7 (Agenda for Change) or equivalent - minimum 3yearsExtensive experience of senior managementExperience in team management including performance managementExperience of receiving and delivering supervisionAble to teach in formal and informal settingsBudget ManagementProven ability to prioritise and manage own workload with minimal supervisionExperience of initiating and implementing changes to servicesExperience of working collaboratively with a variety of agencies and professionals and across organisational boundaries.Evidence of innovation in practice evaluation and audit skills.
