Community Psychiatric Nurse

Job Title: Mental Health Nurse – Older Adults Community Mental Health Team Tripod Partners are currently recruiting for a Mental Health Nurse to join the Older Adults Community Mental Health Team in Preston. This is a full-time role, working Monday to Friday, 9:00-17:00. Key Responsibilities * Manage a caseload of older adults with a range of mental health issues, co-morbidities, and complex presentations. * Conduct assessments, reviews, interventions, and manage discharges, completing all necessary documentation. * Administer depot injections and participate in multi-disciplinary reviews for both community-based patients and inpatients on Mental Health Act sections. * Liaise with members of the multi-disciplinary team and external agencies to ensure comprehensive patient care. * Proficiency in using RiO documentation is required.
