Intermediate Care Team Social Worker - Somerset

Somerset Council are recruiting for an Adult Social Worker to join their immediate care team. This roles is for a full time locum working 37 hours a week, the role allows hybrid working with two days a week in the office. The Role: This role will be two days on site at various locations within East Somerset, the rest of the role can be performed remotely. Case loads are estimated around 20 and you will report into one team manager. Travel will be required to various units to complete necessary work to enable discharge.
